You need to change the connection string with the name of the sql server you want to monitor, the password of the sa user and then implement the nrpe command and service on Nagios.
I use to execute this check only once per day in the morning.

define service{
use generic-service
host_name mysqlserver
servicegroups SqlAgentJobs
service_description Check SQL Agent Jobs on Failure
check_command check_sqlagent_jobs
check_period goodtime
contact_groups sqlmail
notifications_enabled 1
notification_interval 1440
notification_period goodtime
notification_options c,u
}
define command{
command_name check_sqlagent_jobs
command_line $USER1$/./check_nrpe -H $HOSTADDRESS$ -n -p 5666 -t 60 -c check_sql_jobs
}

you have to define in timeperiods.cfg the interval goodtime or use your own interval.
