Changes required in check_apache2.php:-
1. Remove dot (.) from $opt and $def lines as mentioned by Davidak
2. Add question mark (?) to the php open tag at the beginning of file.

The plugin works well for plotting graphs. But I couldn't find a way to set warning and critical limits for say "number of busy workers". I am interested in receiving a Nagios alert when it reaches N number of busy workers.

NRPE used to work with Nagios 3.0 in our environment. But ever since we upgraded to Nagios core 4.0.7 NRPE started giving error:
Aug 13 08:55:25 web1 xinetd: START: nrpe pid=4422 from=::ffff:184.108.40.206
Aug 13 08:55:25 web1 xinetd: FAIL: nrpe address from=::ffff:220.127.116.11
Aug 13 08:55:25 web1 xinetd: EXIT: nrpe status=0 pid=4422 duration=0(sec)
Note: IP is allowed in xinetd config.
