check_apache2.sh
Along with the Array definition problem which davidak has pointed out on Aug 11, 2014, I found a question mark (?) missing in the php template file. After adding this question mark in PHP tag the pnp4nagios graph are populated.
Changes required in check_apache2.php:-

1. Remove dot (.) from $opt[2] and $def[2] lines as mentioned by Davidak
2. Add question mark (?) to the php open tag at the beginning of file.

The plugin works well for plotting graphs. But I couldn't find a way to set warning and critical limits for say "number of busy workers". I am interested in receiving a Nagios alert when it reaches N number of busy workers.

NRPE - Nagios Remote Plugin Executor
Why does this page says compatible with upto Nagios 3.0?

NRPE used to work with Nagios 3.0 in our environment. But ever since we upgraded to Nagios core 4.0.7 NRPE started giving error:
Aug 13 08:55:25 web1 xinetd[6076]: START: nrpe pid=4422 from=::ffff:210.21.7.1
Aug 13 08:55:25 web1 xinetd[4422]: FAIL: nrpe address from=::ffff:210.21.7.1
Aug 13 08:55:25 web1 xinetd[6076]: EXIT: nrpe status=0 pid=4422 duration=0(sec)
Note: IP is allowed in xinetd config.
